These are the terms we use when offering you a choice of either an auto cashback, free gift or cashback by redemption deal on selected mobile purchases from our online store.

How to claim your Phonebox Auto Cashback.

All Phonebox auto cashback claims must be made within 120 days from point of connection, any claims received outside of this period will be considered null and void. We will endeavor to process your auto cashback claim within 5 working days upon receipt of a valid claim and correct documentation.

To claim your auto cashback offer you must provide us with the following details:

• Your first month’s mobile phone bill, showing a completed payment
• 2 forms of ID from the list as stated below
• Your auto cashback voucher (Found on your order confirmation email)

How to claim your Phonebox Cashback by Redemption.

12 Month contract deal redemptions are completed in 3 stages after bills 4, 8 and 12.
18 Month contract deal redemptions are completed in 4 stages after bills 4, 8, 12, and 18.
24 Month contract deal redemptions are completed in 6 stages after bills 4, 8, 12, 16, 20 and 24.

Phonebox Cashback by Redemption refunds will be paid as per the schedule above, All claims should be received within 60 days of the time period as specified. Any claims received outside of these times will be considered null and void. In addition each bill must show that you have remained on the original, connected tariff and paid on time.

To claim your Cashback by Redemption you must provide the following:

• 2 forms of ID from the list as stated below
• Your Redemption voucher - http://www.phoneboxmobiles.co.uk/pdf/cashbackclaimforms.pdf, please follow the claim breakdown on what percentage you need to claim each time.

In addition to the above you will need to provide the mobile airtime bills (or high quality facsimiles), for the first claim you will need to provide the first 4 airtime bills, for each claim subsequently you will need to provide 3 airtime bills e.g, for bill 12 claim you will need to provide airtime bills 12,11 & 10.

How to claim your Phonebox Free Gift.

All Phonebox free gift claims must be made within 60 days from point of connection, any claims received outside this period will be considered null and void. Allow 5-7 days for delivery of gift after claim date.

* All gifts once claimed are shipped directly from our designated suppliers, (ie, Argos, Curry’s, Game etc..) and will require a signature upon delivery. Any warranty issues will be dealt directly with the supplier*

To claim your free gift you must provide the following:

• 2 forms of ID from the list as stated below
• Your Free gift voucher (sent with phone)

Note: All free gift offers are subject to stock availability, should your chosen gift go out of stock between receiving your mobile order and return of your claim form, we will endeavor to deliver your chosen gift within 28 days upon receipt of the claim. If for any the reason the gift becomes no longer available for despatch we reserve the right to replace it with the cash alternative to the value of your chosen gift.

Here are your Proof of Identification Options (we will need one proof from each section)

1. Proof of signature (all valid and up to date)

• UK or International passport
• Passport Extensions are not valid
• All Information must be clear and visible.
• Full or Provisional drivers license (new style picture card), but only if you are not using it as a proof of address.
• Old Style FULL UK drivers license (Old Style Provisional and any Non UK Licences are Not Valid

2. Proof of Address

• Full or Provisional drivers license (New Two Part version only – we need a copy of the photo card)
• Old Style Full UK drivers License (copy the front part only; we cannot accept old style Provisional Licenses
• All Information must be clear and visible.

Please do not send ORIGINAL documentation as a High Quality Facsimile will do just fine.
Remember you cannot use the same piece of ID as both your proof of address and your proof of signature.

These terms and conditions apply to all claims made after the 15th April 2009, for all deals. Documents sent by fax or email will not be acceptable and we will not be able to return any documents sent. We advise that all correspondence be sent by Royal Mail Special Delivery.

All successful claims will be paid within 30 days of receipt.
